What is the IBH Model?
The IBH Model is designed to help Medicaid and Medicare patients manage their mental health conditions by connecting them with necessary physical, behavioral, and social support. This integrated approach is expected to improve the overall quality of care and health outcomes.

These teams will include behavioral and physical health providers, as well as community support services. The IBH Model is set to launch in Fall 2024 and will be tested over eight years in up to eight states. CMS will release a Notice of Funding Opportunity in Spring 2024 for organizations interested in participating in this initiative.
